This is the first time we use electronic form to take order so we can reduce administration workload and continue to provide the service. You can find order form with attached or at CLSC school website:

1. Fill-in Quantity field on the form. Total will be calculated automatically .
2. Click E-Mail Form. You may directly send out the order form from MS outlook, or save the form and send it out with an email to richkuan@hotmail.com
3. Click Print Form and print a copy.
4. Bring this form & payment to pick up your order on 03/22.
